CASE STUDY 2

A 46-year-old, 230lb woman with a family history of breast cancer. She is up to date on yearly mammograms. She has a history of HTN. She complains of hot flushing, night sweats, and genitourinary symptoms.
She had felt well until 1 month ago and she presented to her gynecologist for her annual gyn examination and to discuss her symptoms.
She has a history of ASCUS about 5 years ago on her pap, other than that, Pap smears have been normal. Home medications are Norvasc 10mg qd and HCTZ 25mg qd. Her BP today is 150/90. She has regular monthly menstrual cycles. Her LMP was 1 month ago.

Health Needs

The patient is presenting with peri-menopausal symptoms.  According to DELAMATER & SANTORO, (2018), it involves three stages; premenopausal, menopausal transition and early postmenopause.The patient has a family history of breast cancer which increases her risk of developing the disease. She will therefore require frequent cancer screening for early detection. She is also a known hypertensive on medications, however on physical examination, her blood pressure is elevated. It is important to inquire about compliance to medications and make the necessary dose adjustment.

Patient Education

Encourage on lifestyle changes to maintain a healthy weight. According to Alkabban FM; Ferguson T, (2021), taking part in physical activities is considered as a protective factor against breast cancer incidence by reducing the exposure to endogenous sex hormone therefore altering the immune system response. In addition, physical activity also reduces the chances of obesity which is associated with a greater risk of breast cancer.

With the familial history of breast cancer, educate the patient on the importance of avoiding use of exogenous hormones during the post-menopausal period. According to Alkabban FM; Ferguson T, (2021), hormone replacement therapy increases the risk of breast cancer.

Advise the patient on the importance of frequent breast cancer screening for early detection. First degree relatives of patients with breast cancer have an increased likelihood of developing the disease.

Pharmacotherapeutics

The patient has a familial history of breast cancer and this increases the risk of the disease. Therefore, hormonal replacement therapy is not the mainstay treatment modality in management of the peri-menopausal symptoms. According to Li et al., (2016), black cohosh is a herbal treatment modality that has been shown to reduce hot flushes and mood disturbances. Research studies have also shown that antidepressants such as SSRIs and SNRI have been noted reduce hot flushes.
